LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in home program according to an individual Participant's needs.

Manager of Outreach and Enrollment
Will be responsible for all function related to both Marketing/Outreach activities as well as the assessment and enrollment process for the organization. Candidate must possess the ability to conduct comprehensive evaluation of current marketing/outreach activities, identifying and analyzing trends and developing/implementing strategic initiatives to increase not only the visibility in the community, but ultimately enrollment in LIFE Pittsburgh’s programs and services. In addition, will be responsible for managing the initial assessment process, eligibility determinations and enrollment into the LIFE Pittsburgh program. Candidate must have a bachelor’s degree in related field (Masters preferred), 3-5 years progressive experience with outreach and enrollment with the elderly population, 1-3 years management experience and working knowledge of community based or aging programs/services.
Will schedule appointments for the Participants of LIFE Pittsburgh, including scheduling transportation, obtaining appropriate documentation to send to the appointment ensuring appointment occurs. Candidate must have good organization skills, customer service skills, attention to detail and knowledge of Microsoft Word and Excel. HS Diploma and 1-2 yrs. experience in patient scheduling experience required.
The Dietary Aide will assist the dietitian and the cook with the coordination and serving of nutritious and therapeutic meals and snacks for the elderly participants of LIFE Pittsburgh’s McKee’s Rocks Adult Day Health Center. Ensures all meals served conform to prescribed diet orders and meet the standards for quality and safety. Experience in a healthcare dietary department preferred. Candidate will possess specific knowledge of therapeutic diets including portion size, knowledge of infection control standards and practices and the ability to follow through with direction given by the Dietitian. High School diploma or equivalent. Minimum 1 year experience in a health care setting or food service operation. Monday-Friday daylight position.
Under the direction of the Center Administrator, responsible for promoting & maintaining the physical mobility & well‐being for Participants both in the Center & at home if needed. Will have strong understanding of strengthening, balance, gait training and transfer training. Conducts assessments, treatments, & provides teaching/counseling to Participants & family members. Will coordinate & mentor restorative aides, center health aides & home care aides to support restorative care plans. Must be a graduate of an accredited Physical Therapy Program; must possess a current/valid Commonwealth of Pennsylvania Physical Therapy License, a valid driver’s license with access to a vehicle and minimum of (1) year diversified clinical experience with the geriatric population also required.
Responsible for the regional supervision, coordination and facilitation of transportation services as provided by LIFE Pittsburgh drivers. Responsible for successful on-boarding of new employees, initial and annual competency evaluations and ongoing reinforcement training. This position will have a high expectation to ensure optimal safety of vehicle operations and driver accountability through continual simulated activities, “live” on road assessments, video/camera review, incident/accident investigations and Participant Caregiver interviews to ensure the highest level of competence and safety. Responsible for all HR related functions including, but not limited to Interviewing/hiring, performance management and payroll. Works in close collaboration with the Director or Transportation, Transportation Scheduling Supervisor, Center Administrators, Interdisciplinary Team and Administration to provide safe, efficient and quality transportation services for LIFE Pittsburgh Participants and pre-enrollees. Candidate must have prior supervisory management experience. Candidate must also have 3-5 years’ experience with Geriatric or special needs transportation or safety management, demonstrated experience with competency of safety requirements for transporting the frail and elderly population including proper restraints and working with a variety of functional and cognitive abilities. Bachelor’s degree in applicable field strongly preferred.
The Recreation Assistant will assist our Therapeutic Recreation Specialist in the planning, organizing and implementation of activities in our Adult Day Health Center. This is a Monday through Friday, daylight position. At least 1 yr of experience in a Recreation Department with the elderly or frail or minimum 2 years experience in a health care setting working with personal care within the frail or elderly population and demonstrated ability or training in recreation required.
Assessment and Enrollment Specialist
Reporting to the Manager of outreach and enrollment, the Enrollment Specialist will be the point person to schedule and conduct in-home assessments to educate interested individuals (enrollees) and to preliminary assess for eligibility for the LIFE Pittsburgh program. Once assessed, will coordinate and support the enrollee through the eligibility process to include Clinical Eligibility determination through the Area Agency on Aging and supporting the application for Medical Assistance through the County Assistance Office. Will assist with the coordination with the Inter-Disciplinary team during further assessments, care planning and ultimately, the final enrollment meeting. Will maintain positive relationships with external financial and social service entities to greater support enrollees during enrollment process. May at times have to work non-traditional hours depending on the unique need of the enrollee. May at times assist with the promotion and education of the LIFE Pittsburgh program to potential enrollees in community and outreach events. Bachelor’s degree is Human Service, Social work or related field required. Must have demonstrated experience working with the geriatric population. This position will be based out of our Greentree Administrative office but will frequently travel in the community within our service area.
Community Care Supervisor
Will support the allocation and delivery of personal care services of our Participants in the communities they live. Will provide and ensure that superior customer service is provided to Participants, family members, building management, etc. In collaboration with the Interdisciplinary Team, will identify potential customer service issues and assist in steps to correct and resolve. Responsible for the overall management of the Community Care Coordinators and Personal Care staff to include, but not limited to, interviewing/selection, orientation, training, performance management, payroll, time keeping and regulatory compliance. Certification as a Certified Nursing Assistant or Licensed Practical Nurse (strongly preferred). Valid Driver’s license and automobile insurance are required. Minimum of 3-5 years experience as a Nursing Assistant, Home Care services or other comparable/demonstrated experience, 1-3 years in a progressive management/supervisory role also preferred. Must have HS Diploma or equivalent, Associates or Bachelors degree preferred.
Under the direction of the Center Administrator, the Administrative Assistant will provide a wide range of support to the Center Administrator and Operations Manager to support the organization and functions of the Adult Day Health Center (ADHC) at our Allegheny Center location. Responsible for coordinating and scheduling meetings, preparing materials, prepares correspondence and reports, memos/letters, general office management and other administrative duties. 1-3 years experience in an administrative support roll, Associates Degree or equivalent strongly preferred.
Licensed Practical Nurse
Will assist with all functional nursing activities to include but not limited to medication administration, treatments, wound care and care plan preparedness. Pennsylvania LPN license and one year of experience with the geriatric population required.
Responsible for providing safe transportation for LIFE Pittsburgh Participants in a timely, competent manner per established protocols and procedures. Ensures participant safety on the vehicle including the use of safety belt restraints, wheelchair and object securement, and monitoring of safe transport behaviors. Responsible for vehicle inspections for safe operations and records. Ensures adherence to scheduled transports for safe and timely movement of participants to and from the center, to external appointments and other locations as indicated. Responsible for strong communication with interdisciplinary team and transportation department to ensure optimal safety, care and service. Valid High School Diploma Required. Must have possession of valid PA driver’s license and a clean driving record from prior two years. One year of experience transporting geriatric, frail or disabled passengers is required.
Registered Nurse $10,000 Sign On Bonus
Under the direction of the Nursing Manager, responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will promote and maintain the health of LIFE Pittsburgh participants in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Must have minimum of two years’ experience providing direct nursing care and oversight of Geriatric patients, valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting.
THIS POSITION OFFERS A $10,000 SIGN-ON BONUS, and extra 40 hours of PTO upon hire.
Personal Care Assistant $1,000 Sign On Bonus
Under the direction of the Community Care Supervisor and Community Care Manager, PCA I and PCA II (CNA) will be responsible for providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL’s. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. This position must have a Valid PA driver’s license, with access to a vehicle and one year of experience as a Nursing Assistant, Home Health Aide is required (CNA’s preferred).
THIS POSITION OFFERS A $1,000 SIGN-ON BONUS
Primarily responsible for the management and oversight of professional nursing assessment to determine health needs for individuals enrolled with LIFE Pittsburgh, care plan development and review, and monitoring of nursing interventions. This includes providing direction and evaluation of nursing performance, and adherence to established standards, regulations, policies and procedures to ensure provision of safe, quality care. The Nurse Manager determines staff schedule requirements on a 24 hour coverage basis and works collaboratively with the Day Center Manager to facilitate coordination of services. The Nurse Manager also provides center leadership for all members of the team, in the absence of the Day Center Manager. RN license, three years of experience working with geriatric patients in a health facility or community based setting and 1 to 3 years in a progressive management position required. BS in Nursing or health related profession preferred.
