LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in home program according to an individual Participant's needs.

Health Information Coordinator
Under the direction of the Health Information Manager, the Health Information Coordinator is responsible for ensuring the completion of multiple routine and episodic projects within the Health Information Department for a multi-facility PACE program. Will be responsible for completing, organizing and tracking Health Information reports and tasks. Will work in collaboration with the Health Information Assistants to ensure an accurate, compliant, and complete health information records as needed. Must have two years of experience in medical records (EHR preferred), a driver’s license, access to a vehicle and proficient knowledge of Microsoft Excel. Bachelor’s degree in a healthcare related field strongly preferred, two years of experience in collecting, compiling and analyzing data and report running experience all preferred.
Human Resources Assistant
This position will provide administrative support to all functional areas of HR including Payroll, Recruitment, Employee Relations and general department functions. Working in our Administrative Office in Greentree, duties will include but are not limited to the following:
- Preparing, maintaining and ongoing monitoring of employee active & terminated files, assuring accuracy, compliance with all applicable laws and confidentiality.
- Maintaining and/or distributing various Human Resources related materials including benefit information, summary plan descriptions, employee policy manuals, recruitment information, etc.
- Processes pre-employment hiring function including background checks, references, sanction checks, FBI clearance, license verifications, education verifications, etc.
- May assist with phone screens with applicants for vacant positions that recruiter(s) are working to fill.
- Assists with on-boarding, preparation of new hire packets, including all employment paperwork and benefit forms and assists with new employee orientation.
- Maintains the HRIS with the entry and process of all PAF’s including, new hire information, employee change requests, status and pay rate adjustments, and compiles reports from database as needed.
- Maintains and monitors all staff expirations (Physicals, Insurances, etc), notifying and reminding supervisors and staffs, ensures staff compliance.
- Responds to 3rd party requests for information including verification of employment
- Responsible for various data entry functions for tracking and reporting.
- Provides superior customer service functions by answering employee requests and questions, and investigate in conjunction with HR staff solutions regarding general HR and benefit related issues.
- Performs clerical functions as needed, i.e., photo copying, faxing, filing, etc.
To qualify for this position, the candidate must have excellent customer service skills, experience with Microsoft Office and excellent communication skills with all levels of staff. Candidate must possess formal HR training or education in a related field (BA strongly preferred).

Reporting to the Center Administrator at our Allegheny Center location and within an Interdisciplinary Team setting, the Occupational Therapist is responsible for promoting and maintaining the physical mobility and well-being for Participants both in the Center and at home if needed. Conducts assessments, treatments, and provides teaching and counseling to Participants and family members. Will coordinate and mentor restorative aides, center health aides and home care aides to support restorative care plans. Minimum of (1) year diversified clinical experience with the geriatric population and their family members. Must be a graduate of an accredited Occupational Therapy Program and possess current/valid Commonwealth of Pennsylvania Occupational Therapy License. Also required is a Valid Driver’s license and access to a vehicle.
Licensed Practical Nurse (LPN)
The LPN will assist with all functional nursing activities to include but not limited to medication administration, treatments, wound care and care plan preparedness. Pennsylvania LPN license and one year of experience with the geriatric population required.
Will coordinate & facilitate transportation services for the Participants of LIFE Pittsburgh by maintaining all aspects of the computerized transportation scheduling on a daily basis. Must possess a strong knowledge of handicapped vehicle capabilities and configurations with regard to ambulation and wheelchair dependent participants. Will have a strong understanding of participant demographics, geographical service areas, center visits, medical appointments, and ongoing changes to the schedule to ensure optimal participant care, quality and efficiency. Will work closely with the interdisciplinary team and overall transportation department regarding scheduling, efficiency, noted trends, and participant concerns. This position requires strong organizational and interpersonal skills with the ability to provide positive customer service results in a fast paced environment. HS diploma (Bachelor’s degree preferred) and experience with the geriatric population required. Transportation coordination/scheduling and experience with Microsoft Application programs experience is strongly preferred.
Under the direction of the Center Administrator, the Administrative Assistant will provide a wide range of support to the Center Administrator and Operations Manager to support the organization and functions of the Adult Day Health Center (ADHC) at our North Shore location. Responsible for coordinating and scheduling meetings, preparing materials, prepares correspondence and reports, memos/letters, general office management and other administrative duties. 1-3 years experience in an administrative support roll, Associates Degree or equivalent strongly preferred.
The Dietary Aide will assist the dietitian and the cook with the coordination and serving of nutritious and therapeutic meals and snacks for the elderly participants of LIFE Pittsburgh’s McKee’s Rocks Adult Day Health Center. Ensures all meals served conform to prescribed diet orders and meet the standards for quality and safety. Experience in a healthcare dietary department preferred. Candidate will possess specific knowledge of therapeutic diets including portion size, knowledge of infection control standards and practices and the ability to follow through with direction given by the Dietitian. High School diploma or equivalent. Minimum 1 year experience in a health care setting or food service operation. Monday-Friday daylight position.
Under the direction of the Nursing Manager, the RN is responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will promote and maintain the health of LIFE Pittsburgh participants in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Must have minimum of two years’ experience providing direct nursing care and oversight of Geriatric patients, valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting.
Manager of Outreach and Enrollment
Will be responsible for all function related to both Marketing/Outreach activities as well as the assessment and enrollment process for the organization. Candidate must possess the ability to conduct comprehensive evaluation of current marketing/outreach activities, identifying and analyzing trends and developing/implementing strategic initiatives to increase not only the visibility in the community, but ultimately enrollment in LIFE Pittsburgh’s programs and services. In addition, will be responsible for managing the initial assessment process, eligibility determinations and enrollment into the LIFE Pittsburgh program. Candidate must have a bachelor’s degree in related field (Masters preferred), 3-5 years progressive experience with outreach and enrollment with the elderly population, 1-3 years management experience and working knowledge of community based or aging programs/services.
Will schedule appointments for the Participants of LIFE Pittsburgh, including scheduling transportation, obtaining appropriate documentation to send to the appointment ensuring appointment occurs. Candidate must have good organization skills, customer service skills, attention to detail and knowledge of Microsoft Word and Excel. HS Diploma and 1-2 yrs. experience in patient scheduling experience required.
Responsible for the day to day operations of the Greentree Adult Day Health Center, administration and coordination of care delivered as described in the individual Plan of Care for each LIFE Pittsburgh Participant. They will facilitate and coordinate the Interdisciplinary Team care planning process to ensure decisions are guided by the Participant’s goals of care and are consistent with the PACE model concepts. The Center Administrator is responsible for management, leadership, development, and accountability of interdisciplinary team members and staff. The Center Administrator is responsible for both participant and center based budget monitoring, spending and analysis of variances that arise. Is responsible for participation and involvement with the LIFE Pittsburgh Quality Program to monitor and ensure optimal operational and participant outcomes. Must have valid Driver’s license and access to a vehicle for possible travel in community. Must have Bachelor’s degree in a healthcare related field, Master’s Degree strongly preferred. Minimum of (1) year experience in a geriatric community setting and experience working with and relating to the geriatric population and their family members required. Prior management experience also required.
The Recreation Assistant will assist our Therapeutic Recreation Specialist in the planning, organizing and implementation of activities in our Adult Day Health Center. This is a Monday through Friday, daylight position. At least 1 yr of experience in a Recreation Department with the elderly or frail or minimum 2 years experience in a health care setting working with personal care within the frail or elderly population and demonstrated ability or training in recreation required.
Responsible for providing safe transportation for LIFE Pittsburgh Participants in a timely, competent manner per established protocols and procedures. Ensures participant safety on the vehicle including the use of safety belt restraints, wheelchair and object securement, and monitoring of safe transport behaviors. Responsible for vehicle inspections for safe operations and records. Ensures adherence to scheduled transports for safe and timely movement of participants to and from the center, to external appointments and other locations as indicated. Responsible for strong communication with interdisciplinary team and transportation department to ensure optimal safety, care and service. Valid High School Diploma Required. Must have possession of valid PA driver’s license and a clean driving record from prior two years. One year of experience transporting geriatric, frail or disabled passengers is required.
Personal Care Assistant $1,000 Sign On Bonus
Under the direction of the Community Care Supervisor and Community Care Manager, PCA I and PCA II (CNA) will be responsible for providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL’s. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. This position must have a Valid PA driver’s license, with access to a vehicle and one year of experience as a Nursing Assistant, Home Health Aide is required (CNA’s preferred).

Community Care Supervisor
will support the allocation and delivery of personal care services of our Participants in the communities they live. Will provide and ensure that superior customer service is provided to Participants, family members, building management, etc. In collaboration with the Interdisciplinary Team, will identify potential customer service issues and assist in steps to correct and resolve. Responsible for the overall management of the Community Care and Personal Care staff to include, but not limited to, interviewing/selection, orientation, training, performance management, payroll, time keeping and regulatory compliance. Experience in supervision preferred. Certification as a Certified Nursing Assistant or Licensed Practical Nurse (strongly preferred).
