At LIFE Pittsburgh, we offer more than a collection of services. We provide comprehensive care shaped around you, helping you continue to live safely and meaningfully at home.
A National Model, A Local Mission
The Program of All-Inclusive Care for the Elderly (PACE) provides seniors with an alternative to nursing facilities and enables them to live in their homes.
Known as Living Independence for the Elderly (LIFE) in Pennsylvania, a team of health care professionals works with each individual to make sure needed services are coordinated. LIFE programs get to know their Participants and design services to meet each person’s unique needs.

Who We Are
LIFE Pittsburgh is a nonprofit organization and one of the earliest adopters of the PACE model in Pennsylvania. For nearly 30 years, we have supported thousands of older adults in Allegheny County.
Our roots in the community run deep. As a mission-driven organization, we exist to support older adults and their families through an approach to care that is personal, comprehensive, and centered on helping each individual continue living life at home and in the community.
Behind every LIFE Pittsburgh story is a simple belief: aging happens best at home, where life feels familiar, meaningful, and connected.
Your care plan is made for you. It reflects your goals, strengths, daily life, and what matters most to you. Whether you’re at home, in the hospital, in rehab, or at one of our Health and Wellness Centers, your Interdisciplinary Team (IDT) stays involved in your care.
Create a care plan shaped by each participant’s needs and goals.
Provide coordinated medical care, nursing, therapy, and other services at home and in our Health and Wellness Centers.
Arrange transportation and other practical support.
Help participants live safely at home for as long as possible.
Support social connections and participation in community life.

After Hours Medical
On-Call Care
Our Health & Wellness Centers are open Monday‑Friday 8a‑4:30p to meet your needs.
For clinical needs after hours during the week, on weekends and holidays, support is just a phone call away.
Transportation
Transportation to all medical appointments and to and from the Health & Wellness Center is provided.
We accommodate special transportation needs and will help you get ready or settled back at home before we leave.
LIFE Pittsburgh assists and supports seniors throughout our approved service areas in Allegheny County, Pennsylvania.
What Will I Pay?
The amount you pay for LIFE Pittsburgh depends on whether you have Medicare, Medicaid, both, or neither. In Pennsylvania, Medicaid is also called Medical Assistance.
If you have both Medicare and Medicaid: You will not pay a monthly premium, deductible, or copayment.
If you have Medicaid but not Medicare: You will not pay a monthly premium, deductible, or copayment.
If you have Medicare but not Medicaid: You will pay a monthly premium. The amount depends on your Medicare coverage.
If you have neither Medicare nor Medicaid: You may still be eligible to enroll by paying privately for services.
Our Enrollment Team will review your coverage and explain your specific costs before you decide to enroll. If you need to apply for Medical Assistance, we can help you understand the application process and next steps.

Eligibility and Cost
LIFE Pittsburgh serves seniors from all walks of life who meet the following eligibility requirements:
55 years and older
Live in designated zip codes served
Qualify for state nursing facility level of care criteria
Live safely in the community with LIFE services at the time of enrollment
Meet any additional program-specific eligibility conditions imposed under the LIFE program agreement.
LIFE Pittsburgh does not discriminate in the delivery of LIFE services based on race, ethnicity, national origin, religion, sex, sexual orientation, age, mental or physical disabilities, or source of payment.
Participants in LIFE Pittsburgh must receive necessary healthcare—including primary and specialty physician services—from LIFE Pittsburgh or providers contracted with us, except in an emergency. Participants may be responsible for the cost of services not approved by the Interdisciplinary Team (IDT) or obtained outside LIFE Pittsburgh without authorization.
