LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in home program according to an individual Participant's needs.

Under the direction of the Center Administrator, the Administrative Assistant will provide a wide range of support to the Center Administrator and Operations Manager to support the organization and functions of the Adult Day Health Center (ADHC). Responsible for coordinating and scheduling meetings, preparing materials, prepares correspondence and reports, memos/letters, general office management and other administrative duties. 1-3 years experience in an administrative support roll, Associates Degree or equivalent strongly preferred.
Responsible for the day to day operations of the Adult Day Health Center, administration and coordination of care delivered as described in the individual Plan of Care for each LIFE Pittsburgh Participant. They will facilitate and coordinate the Interdisciplinary Team care planning process to ensure decisions are guided by the Participant’s goals of care and are consistent with the PACE model concepts. The Center Administrator is responsible for management, leadership, development, and accountability of interdisciplinary team members and staff. The Center Administrator is responsible for both participant and center-based budget monitoring, spending and analysis of variances that arise. Is responsible for participation and involvement with the LIFE Pittsburgh Quality Program to monitor and ensure optimal operational and participant outcomes. Must have valid Driver’s license and access to a vehicle for possible travel in community. Must have Bachelor’s degree in a healthcare related field, Master’s Degree strongly preferred. Minimum of (1) year experience in a geriatric community setting and experience working with and relating to the geriatric population and their family members required. Prior management experience also required.
Under the direction of the Center Supervisor, the Center Aide will assist Participants with personal care and social needs while attending our Adult Day Health Center. The Center Aide will assist Participants with personal care needs, ambulation, escorting for appointments dining room service, and during recreational activities. Must have High School Diploma or GED and a minimum 1 year working with the geriatric population required.
Community Care Supervisor
The Community Care Supervisor will support the allocation and delivery of personal care services of our Participants in the communities they live. Will provide and ensure that superior customer service is provided to Participants, family members, building management, etc. In collaboration with the Interdisciplinary Team, will identify potential customer service issues and assist in steps to correct and resolve. Responsible for the overall management of the Community Care and Personal Care staff to include, but not limited to, interviewing/selection, orientation, training, performance management, payroll, time keeping and regulatory compliance. Experience in supervision preferred. Certification as a Certified Nursing Assistant or Licensed Practical Nurse (strongly preferred).
Full TIme On Call Registered Nurse $10,000 Sign On Bonus
With responsibility of the care of our Participants 24 Hours, 7 days a week, we are currently searching for a Full Time, Exempt Registered Nurse to work on-call and will be responsible for the telephonic assessment and in person assessment as needed. Responsible for on-call phone calls and communication that occurs involving Participant care through our on-call system. Will be able to utilize clinical knowledge and critical thinking to determine best course of action when receiving on-call phone calls. Ensure high quality care and communication is delivered at all times. Provide direct care to the patients, including administering treatments and medications. Assist in development, follow and coordinate each Participant comprehensive assessment and plan of care. Generally provides triage. Notify physician, when necessary, of changes in the patients condition, weight, medication errors, medication management issues, abnormal laboratory/x-ray results and any other unusual observation. Complete all necessary documentation according to regulations and LIFE Pittsburgh’s policies/procedures. Must have current PA RN license in good standing, minimum of one year of med/surg; home health or hospice nursing experience, be self-directed with excellent observational ability and effective communication skills, have the ability to provide quality written documentation in a timely manner and possess a valid Drivers License and access to a vehicle with current auto insurance.
Housekeeper - $5,000 Sign On Bonus
Responsible for following an approved plan of care for housekeeping services for LIFE Pittsburgh Participants. Will provide a variety of cleaning functions and duties to ensure that Participants homes are well kept and safe for Participants and staff. Experience in housekeeping preferred, Valid PA Drivers License and use of a vehicle required.
Licensed Practical Nurse - $5,000 Sign On Bonus
Will assist with all functional nursing activities to include but not limited to medication administration, treatments, wound care and care plan preparedness. Pennsylvania LPN license and one year of experience with the geriatric population required. This position offers a $5,000 Sign-on Bonus.
Medical Services Assistant
Reporting to the RN manager, the Medical Service Assistant will work directly with the medical services staff to assist doctors and nurses with patient exam and care. Must have valid PA Driver’s License, Minimum 1 year experience providing direct care within the Elderly/Frail Population in a healthcare facility required. Valid PA Certified Nursing Assistant Certification (preferred) or comparable certificate/experience required.
Provider Contracting Specialist
Under the direction of the Director of Accounting, the Provider Contracting Specialist is responsible for administrative activities associated with provider contracting, credentialing and provider relations. Process to include initiating and maintaining contract paperwork and frequent contact with providers and LIFE Pittsburgh staff to ensure compliance with external reporting requirements and internal policies. Must have 2-3 years of contracting paperwork processing or credentialing experience. Knowledge of physician office practices or provider network in managed care environment preferred. Associates degree also preferred.
The Quality Analyst is a dynamic position to support program-wide quality program and compliance with Federal, State and PACE regulations. Reporting to the Manger of Quality, this position supports the quality functions such as the data collection, entry, compilation and initial data analysis. This position works closely with the Manager of Quality, management and clinical/operational staff towards to ensure data integrity and reliability. This role will be responsible for compiling internal reports in collaboration with the Manager of Quality and assist on projects focused on quality improvement. Must possess a valid driver’s license and access to a vehicle. Bachelor Degree required (BSN or Master’s Degree strongly preferred), driver’s license and access to a vehicle, high proficiency in Microsoft office, specifically in Excel including data organization, aggregation, inserting formulas and functions, creating and using pivot tables and creating data visualization etc. as well as proficiency in overall Microsoft Office Suite are all required.
Responsible for assisting our Therapeutic Recreation Specialist in the planning, organizing and implementation of activities in our Adult Day Health Center. This is a Monday through Friday, daylight position. Must have High School Diploma or equivalent, at least 1 yr of experience in a Recreation Department with the elderly or frail or minimum 2 years experience in a health care setting working with personal care within the frail or elderly population and demonstrated ability or training in recreation required.
Under the direction of the Center Administrator, the Rehab Aide is responsible for providing or assisting participants with direct and indirect restorative or rehabilitative care needs under the direct supervision of a licensed therapist, including functional and instrumental activities of daily living and therapeutic activities as determined by the interdisciplinary team and the individual care plan. Under the direction of the Rehabilitation Coordinator, performs restorative care services for participants at their place of residence and/or Adult Day Health Center. Maintains participant rights, privacy, and confidentiality. Assist in general equipment/ inventory maintenance, DME management; assist in scheduling of rehabilitation / restorative / home assessment appointments. Assist in execution of restorative and/or rehabilitation programs under the direction of Occupational / Physical Therapist. A minimum of 1 year experience as a Restorative / Rehabilitation Aide is required, experienced in working with the frail elderly / geriatric population.
Registered Nurse $10,000 Sign On Bonus
Under the direction of the Nursing Manager, the RN is responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will promote and maintain the health of LIFE Pittsburgh participants in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Must have minimum of two years’ experience providing direct nursing care and oversight of Geriatric patients, valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting. Social Worker $5,000 Sign On Bonus
Responsible for the profession’s unique psycho social perspective to the interdisciplinary evaluation, assessment, plan of care, and ongoing services that occur once LIFE Pittsburgh Participants begin the enrollment process. Interventions could include individual Participant contacts; appropriate collateral contact; Participant and family education, assessment and counseling; mobilizing resources, addressing mental health needs as they arise; ongoing care management; advocacy to ensure patient needs are addressed; and in the event, dis-enrollment education and procedures. In partnership with the Interdisciplinary Team, ensures effective, efficient, and appropriate care in order to optimize the health status and quality of life for our Participants.· The Social Worker is knowledgeable regarding social systems and institutions, individual behaviors and can skillfully apply appropriate interventions to meet the needs of LIFE Pittsburgh Participants and family members. Candidate must have Masters in Social Work (MSW), valid PA Driver’s License AND Access to a vehicle with current auto Insurance, minimum of (1) year geriatric experience in a hospital, long-term care facility or community setting. Also must have Experience working with and relating to the geriatric population and their family. This position offers a $5,000 sign on bonus.
Personal Care Assistant/CNA up to $5,000 Sign On Bonus (depending upon shift hired for)
Under the direction of the Community Care Supervisor and Community Care Manager, PCA I and PCA II (CNA) will be responsible for providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL’s. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. This position must have a Valid PA driver’s license, with access to a vehicle and one year of experience as a Nursing Assistant, Home Health Aide is required (CNA’s preferred).

Van Driver $5,000 Sign On Bonus
The Van Driver at LIFE Pittsburgh is responsible for providing safe transportation for LIFE Pittsburgh Participants in a timely, competent manner per established protocols and procedures. Ensures participant safety on the vehicle including the use of safety belt restraints, wheelchair and object securement, and monitoring of safe transport behaviors. Responsible for vehicle inspections for safe operations and records. Ensures adherence to scheduled transports for safe and timely movement of participants to and from the center, to external appointments and other locations as indicated. Responsible for strong communication with interdisciplinary team and transportation department to ensure optimal safety, care and service. Valid High School Diploma Required. Must have possession of valid PA driver’s license and a clean driving record from prior two years. One year of experience transporting geriatric, frail or disabled passengers is required.
