LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in home program according to an individual Participant's needs.

Center Aide

Pittsburgh, PA

Under the direction of the Center Supervisor, the Center Aide will assist Participants with personal and social needs while attending our Adult Day Health Center. Will assist Participants with personal care needs, ambulation, escorting for appointments dining room service, and during recreational activities. HS Diploma, minimum of one year working with the geriatric population is required. This is a M-F DAYLIGHT position.

Eligibility Specialist

Pittsburgh, PA

Full time position working closely with the Director of Eligibility and our Enrollment department with the main responsibility of assisting prospective and current Participants with obtaining, determining and maintaining eligibility for benefits and services.  Will be the point person to assist individuals in the application process for Medical Assistance, which may include home visits for application completion, verification of documents and problem solving.  Completes and coordinates final submission to the Allegheny County Assistance Office and supports Participants with any pending issues, requests and or appeals for financial approval.  Works with IDT members on current enrollees for financial recertification.  Candidate must have experience and be comfortable working with, and in the homes, of the elderly population dealing with sensitive financial issues.  Must be extremely organized and/or flexible with as many as 20-25 open cases at a time.  Must have use of a vehicle for local travel to Participant homes

Human Resources Manager

Pittsburgh, PA

The Human Resources Manager will be responsible for planning, organizing and directing the Human Resources department for a local non-profit healthcare organization to recruit and retain a high quality workforce and to develop an employee and Participant oriented organizational culture that emphasizes quality, continuous improvement and high performance. This is a “hands-on”*, high paced management position. Key responsibilities include the monitoring and developing HR policies and procedures, strong emphasis on employee recruitment and retention, regular support of management with employee relation issues, strategic compensation, bi-weekly payroll processing, performance management, and legal compliance. The Human Resources Manager is a key member of the Management Team, reports to the Senior Director of Operations and will supervise a staff of 5-6. Minimum of 5 years Human Resources “generalist” experience across all functional areas of Human Resources, with experience leading or managing these functions. BA/BS or equivalent required; Master’s Degree, PHR or SPHR certification preferred knowledge of state and federal employment laws, wage and salary rules and regulations. Must have demonstrated ability to function in rapidly growing and fast-paced work environment, ability to recommend HR strategy and develop tactical action and project plans, exercise judgment and take independent action on day-to-day issues.

Licensed Practical Nurse (LPN) - $5,000 Sign on bonus

Registered Dietitian

McKees Rocks, PA

provide LIFE-Pittsburgh Participants medical nutrition therapy based on individual needs and disease states as well as interactions with TEAM to promote coordination of quality care. Will manage the kitchen by adhering to food safety standards/regulations, maintaining cost effectiveness through appropriate ordering of supplies and promoting coordination of Participants’ nutritional needs with the IDT and dietary staff. Bachelor’s degree in Dietetics or appropriate alternative degree plus minimum of 1 year experience in clinical nutrition/assessment in a health care or geriatric setting required. Must be a Registered Dietitian through the Commission on Dietetic Registration. Will obtain/maintain food service certification.

Transportation Supervisor

Pittsburgh, PA

Will be responsible for the regional supervision, coordination and facilitation of transportation services as provided by LIFE Pittsburgh drivers. Responsible for successful on-boarding of new employees, initial and annual competency evaluations and ongoing reinforcement training. This position will have a high expectation to ensure optimal safety of vehicle operations and driver accountability through continual simulated activities, “live” on road assessments, video/camera review, incident/accident investigations and Participant Caregiver interviews to ensure the highest level of competence and safety. Responsible for all HR related functions including, but not limited to Interviewing/hiring, performance management and payroll. Works in close collaboration with the Director or Transportation, Transportation Scheduling Supervisor, Center Administrators, Interdisciplinary Team and Administration to provide safe, efficient and quality transportation services for LIFE Pittsburgh Participants and pre-enrollees. Candidate must have prior supervisory management experience. Candidate must also have 3-5 years’ experience with Geriatric or special needs transportation or safety management, demonstrated experience with competency of safety requirements for transporting the frail and elderly population including proper restraints and working with a variety of functional and cognitive abilities. Bachelor’s degree in applicable field strongly preferred.

Registered Nurse $10,000 Sign On Bonus

Under the direction of the Nursing Manager, the RN is responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will promote and maintain the health of LIFE Pittsburgh participants in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Must have minimum of two years’ experience providing direct nursing care and oversight of Geriatric patients, valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting. Manager of Outreach and Enrollment

Pittsburgh, PA

Will be responsible for all function related to both Marketing/Outreach activities as well as the assessment and enrollment process for the organization. Candidate must possess the ability to conduct comprehensive evaluation of current marketing/outreach activities, identifying and analyzing trends and developing/implementing strategic initiatives to increase not only the visibility in the community, but ultimately enrollment in LIFE Pittsburgh’s programs and services. In addition, will be responsible for managing the initial assessment process, eligibility determinations and enrollment into the LIFE Pittsburgh program. Recreation Assistant

The Recreation Assistant will assist our Therapeutic Recreation Specialist in the planning, organizing and implementation of activities in our Adult Day Health Center. This is a Monday through Friday, daylight position. At least 1 yr of experience in a Recreation Department with the elderly or frail or minimum 2 years experience in a health care setting working with personal care within the frail or elderly population and demonstrated ability or training in recreation required.

Personal Care Assistant $1,000 Sign On Bonus

Under the direction of the Community Care Supervisor and Community Care Manager, PCA I and PCA II (CNA) will be responsible for providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL’s. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. This position must have a Valid PA driver’s license, with access to a vehicle and one year of experience as a Nursing Assistant, Home Health Aide is required (CNA’s preferred).


Van Driver

Pittsburgh, PA

The Van Driver at LIFE Pittsburgh is responsible for providing safe transportation for LIFE Pittsburgh Participants in a timely, competent manner per established protocols and procedures. Ensures participant safety on the vehicle including the use of safety belt restraints, wheelchair and object securement, and monitoring of safe transport behaviors. Responsible for vehicle inspections for safe operations and records. Ensures adherence to scheduled transports for safe and timely movement of participants to and from the center, to external appointments and other locations as indicated. Responsible for strong communication with interdisciplinary team and transportation department to ensure optimal safety, care and service. Valid High School Diploma Required. Must have possession of valid PA driver’s license and a clean driving record from prior two years. One year of experience transporting geriatric, frail or disabled passengers is required.

Help Desk Support Specialist

Pittsburgh, PA

Under the direction of the Manager of Information Systems, will be responsible for providing technical assistance and support to users by answering questions and troubleshooting problems related to computer systems, hardware or software. Will track maintenance schedules and inventory for all equipment and software. Will order, setup, and deliver mobile phones and/or desktops/laptops to appropriate personnel. Must have working knowledge of Windows 7, Windows 10 operating systems, MS Office 2010, 2013, 2016/Office 365, Active Directory and Microsoft Exchange and must also have the ability to troubleshoot complex problems. Candidate will have minimum of a 2 or 4 year technical trade school degree in information technology, Bachelor’s degree or 1-3 years’ experience in a help desk environment strongly preferred.
