LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in home program according to an individual Participant's needs.

Center Aide

Pittsburgh, PA

Under the direction of the Center Supervisor, the Center Aide will assist Participants with personal care and social needs while attending our Adult Day Health Center. The Center Aide will assist Participants with personal care needs, ambulation, escorting for appointments dining room service, and during recreational activities. High School Diploma, minimum of one year working with the geriatric population is required.

Community Care Supervisor

Pittsburgh, PA

The Community Care Supervisor will support the allocation and delivery of personal care services of our Participants in the communities they live. Will provide and ensure that superior customer service is provided to Participants, family members, building management, etc. In collaboration with the Interdisciplinary Team, will identify potential customer service issues and assist in steps to correct and resolve. Responsible for the overall management of the Community Care and Personal Care staff to include, but not limited to, interviewing/selection, orientation, training, performance management, payroll, time keeping and regulatory compliance. Experience in supervision preferred. Certification as a Certified Nursing Assistant or Licensed Practical Nurse (strongly preferred).

Dietary Aide

Pittsburgh, PA

This is a Monday – Friday, Daylight position. The Dietary Aide will assist the dietitian and the cook with the coordination and serving of nutritious and therapeutic meals and snacks for the elderly participants of LIFE Pittsburgh’s McKee’s Rocks Adult Day Health Center. Ensures all meals served conform to prescribed diet orders and meet the standards for quality and safety. Experience in a healthcare dietary department preferred. Candidate will possess specific knowledge of therapeutic diets including portion size, knowledge of infection control standards and practices and the ability to follow through with direction given by the Dietitian. High School diploma or equivalent. Minimum 1 year experience in a health care setting or food service operation.

Education Coordinator

Pittsburgh, PA

The education Coordinator is responsible for the development, implementation and evaluation of the organization’s education needs in collaboration with the Training/Education Manager and Management Team of LIFE Pittsburgh as defined by CMS and Department of Public Welfare regulations. Responsible for education related compliance for all regulatory body reviews to include local, state and federal constituencies. Will implement effective methods to educate, enhance and improve clinical performance. Maintenance of education and training records to be consistent with regulations as necessary for on-sight reviews. Assist with staff development and will respond to identified education needs for clinical and nonclinical staff. Will also provide education programs to respond to areas of deficiencies or in need of improvement. RN and BA required (BSN preferred). Driver’s License, access to vehicle and 3-5 years education/training experience in a health care environment also required.

Facilities Specialist

Pittsburgh, PA

Responsible for ensuring that the LIFE Pittsburgh’s (4) physical locations are safe, well-kept and reflect LIFE Pittsburgh’s expectations. Will perform general maintenance repairs and requests and/or will coordinate with applicable property managers to correct issues in a timely manner. Works with outside vendors on estimates and quotes of significant work/projects and coordinates and oversees work conducted. Will continually evaluate physical locations for area’s that need repaired or renovated. Actively participates on the Safety Committee to assist and ensure locations are safe for Participants and Staff. Will work within the Health Centers, with daily travel to all centers as needed. Vehicle for travel is required. Candidate must have demonstrated knowledge of basic maintenance issues/equipment (lighting, plumbing, etc) and also with minor renovations (carpet cleaning, drywall repair, painting, etc). Must have the ability to prioritize multiple projects at once and establish priorities in order to meet the needs.

Housekeeper

Responsible for following an approved plan of care for housekeeping services for LIFE Pittsburgh Participants.  Will provide a variety of cleaning functions and duties to ensure that Participants homes are well kept and safe for Participants and staff.  Experience in housekeeping preferred, Valid PA Drivers License and use of a vehicle required.

Licensed Practical Nurse

Pittsburgh, PA

The LPN will assist with all functional nursing activities to include but not limited to medication administration, treatments, wound care and care plan preparedness. Pennsylvania LPN license and one year of experience with the geriatric population required.

Operations Manager

Pittsburgh, PA

In collaboration with and under the direction of the Center Administrator, the Operations Manager is responsible for aspects of the day-to-day operations, administration and coordination of care delivered to LIFE Pittsburgh Participants to include ensuring safe and effective operations of the day center, Participant feedback and grievance investigation and completion, environmental and maintenance, participation with quality improvement initiatives and supervision and management of center staff. Candidate must have experience working with and relating to the geriatric population and their family members (prior management preferred), minimum of (1) year experience in a hospital or long term care facility or setting. Must also possess a Bachelor’s degree in a health care related field (Masters degree strongly preferred) or 1-3 years of experience in and Interdisciplinary (IDT) or supervisory role in a PACE environment.

Quality Analyst

Pittsburgh, PA

The Quality Analyst is a dynamic position to support program-wide quality program and compliance with Federal, State and PACE regulations. Reporting to the Manger of Quality, this position supports the quality functions such as the data collection, entry, compilation and initial data analysis. This position works closely with the Manager of Quality, management and clinical/operational staff towards to ensure data integrity and reliability. This role will be responsible for compiling internal reports in collaboration with the Manager of Quality and assist on projects focused on quality improvement. Must possess a valid driver’s license and access to a vehicle. Bachelor Degree required (BSN or Master’s Degree strongly preferred), driver’s license and access to a vehicle, high proficiency in Microsoft office, specifically in Excel including data organization, aggregation, inserting formulas and functions, creating and using pivot tables and creating data visualization etc. as well as proficiency in overall Microsoft Office Suite are all required.

Receptionist

Performs reception and clerical duties at our Allegheny Center location, that include high telephone call volume; opens, dates and separates mail; and greets and directs participants, visitors, and/or vendors to the appropriate departments. Ensures operation and organization of the reception area. Provides clerical support, as needed while demonstrating and reflecting a commitment to the mission and core values of LIFE Pittsburgh to assist the elderly in maintaining their dignity and independence in the community. Must have excellent communication and interpersonal skills, ability to maintain composure in a high-volume, multi-focused work environment, knowledge of multiple computer software applications, (ie. MS Office) operate standard office machines and equipment such as multi-line telephone calculator, fax, and copier. 1-3 year experience in a fast paced, multi-tasking administrative role required.

Registered Nurse $10,000 Sign On Bonus

Under the direction of the Nursing Manager, the RN is responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will promote and maintain the health of LIFE Pittsburgh participants in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Must have minimum of two years’ experience providing direct nursing care and oversight of Geriatric patients, valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting. Registered Dietitian

Pittsburgh, PA

The Registered Dietitian will provide LIFE-Pittsburgh Participants medical nutrition therapy based on individual needs and disease states as well as interactions with TEAM to promote coordination of quality care. Will manage the kitchen by adhering to food safety standards/regulations, maintaining cost effectiveness through appropriate ordering of supplies and promoting coordination of Participants’ nutritional needs with the IDT and dietary staff. Bachelor’s degree in Dietetics or appropriate alternative degree plus minimum of 1 year experience in clinical nutrition/assessment in a health care or geriatric setting required. Must be a Registered Dietitian through the Commission on Dietetic Registration. Will obtain/maintain food service certification.

Rehabilitation Aide

Pittsburgh, PA

Under the direction of the Center Administrator, the Rehab Aide is responsible for providing or assisting participants with direct and indirect restorative or rehabilitative care needs under the direct supervision of a licensed therapist, including functional and instrumental activities of daily living and therapeutic activities as determined by the interdisciplinary team and the individual care plan. Under the direction of the Rehabilitation Coordinator, performs restorative care services for participants at their place of residence and/or Adult Day Health Center. Maintains participant rights, privacy, and confidentiality. Assist in general equipment/ inventory maintenance, DME management; assist in scheduling of rehabilitation / restorative / home assessment appointments. Assist in execution of restorative and/or rehabilitation programs under the direction of Occupational / Physical Therapist. A minimum of 1 year experience as a Restorative / Rehabilitation Aide is required, Experienced in working with the frail elderly / geriatric population.

Social Worker

Pittsburgh, PA

Responsible for the profession’s unique psycho social perspective to the interdisciplinary evaluation, assessment, plan of care, and ongoing services that occur once LIFE Pittsburgh Participants begin the enrollment process. The Social Worker interventions could include individual Participant contacts; appropriate collateral contact; Participant and family education, assessment and counseling; mobilizing resources, addressing mental health needs as they arise; ongoing care management; advocacy to ensure patient needs are addressed; and in the event, dis-enrollment education and procedures. The Social Worker, in partnership with the Interdisciplinary Team, ensures effective, efficient, and appropriate care in order to optimize the health status and quality of life for our Participants. The Social Worker is knowledgeable regarding social systems and institutions, individual behaviors and can skillfully apply appropriate interventions to meet the needs of LIFE Pittsburgh Participants and family members. Must possess a valid Driver’s license and automobile insurance, Master’s Degree in Social Work, minimum of (1) year geriatric experience in a hospital, long-term care facility or community setting. Must also have experience working with and relating to the geriatric population and their family members.

Personal Care Assistant $1,000 Sign On Bonus

Under the direction of the Community Care Supervisor and Community Care Manager, PCA I and PCA II (CNA) will be responsible for providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL’s. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. This position must have a Valid PA driver’s license, with access to a vehicle and one year of experience as a Nursing Assistant, Home Health Aide is required (CNA’s preferred).

Van Driver

Pittsburgh, PA

The Van Driver at LIFE Pittsburgh is responsible for providing safe transportation for LIFE Pittsburgh Participants in a timely, competent manner per established protocols and procedures. Ensures participant safety on the vehicle including the use of safety belt restraints, wheelchair and object securement, and monitoring of safe transport behaviors. Responsible for vehicle inspections for safe operations and records. Ensures adherence to scheduled transports for safe and timely movement of participants to and from the center, to external appointments and other locations as indicated. Responsible for strong communication with interdisciplinary team and transportation department to ensure optimal safety, care and service. Valid High School Diploma Required. Must have possession of valid PA driver’s license and a clean driving record from prior two years. One year of experience transporting geriatric, frail or disabled passengers is required.
