LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in home program according to an individual Participant's needs.

The Center Administrator is responsible for the day to day operations of the Adult Day Health Center, administration and coordination of care delivered as described in the individual Plan of Care for each LIFE Pittsburgh Participant. They will facilitate and coordinate the Interdisciplinary Team care planning process to ensure decisions are guided by the Participant’s goals of care and are consistent with the PACE model concepts. The Center Administrator is responsible for management, leadership, development, and accountability of interdisciplinary team members and staff. The Center Administrator is responsible for both participant and center based budget monitoring, spending and analysis of variances that arise. Is responsible for participation and involvement with the LIFE Pittsburgh Quality Program to monitor and ensure optimal operational and participant outcomes. Must have valid Driver’s license and access to a vehicle for possible travel in community. Must have Bachelor’s degree in a healthcare related field, Master’s Degree strongly preferred. Minimum of (1) year experience in a geriatric community setting and experience working with and relating to the geriatric population and their family members required. Prior management experience also required.
Under the direction of the Center Supervisor, the Center Aide will assist Participants with personal and social needs while attending our Adult Day Health Center. Will assist Participants with personal care needs, ambulation, escorting for appointments dining room service, and during recreational activities. HS Diploma, minimum of one year working with the geriatric population is required. This is a M-F DAYLIGHT position.
Human Resources Manager
The Human Resources Manager will be responsible for planning, organizing and directing the Human Resources department for a local non-profit healthcare organization to recruit and retain a high quality workforce and to develop an employee and Participant oriented organizational culture that emphasizes quality, continuous improvement and high performance. This is a “hands-on”*, high paced management position. Key responsibilities include the monitoring and developing HR policies and procedures, strong emphasis on employee recruitment and retention, regular support of management with employee relation issues, strategic compensation, bi-weekly payroll processing, performance management, and legal compliance. The Human Resources Manager is a key member of the Management Team, reports to the Senior Director of Operations and will supervise a staff of 5-6. Minimum of 5 years Human Resources “generalist” experience across all functional areas of Human Resources, with experience leading or managing these functions. BA/BS or equivalent required; Master’s Degree, PHR or SPHR certification preferred knowledge of state and federal employment laws, wage and salary rules and regulations. Must have demonstrated ability to function in rapidly growing and fast-paced work environment, ability to recommend HR strategy and develop tactical action and project plans, exercise judgment and take independent action on day-to-day issues.
Licensed Practical Nurse (LPN) - $5,000 Sign on bonus
The LPN will assist with all functional nursing activities to include but not limited to medication administration, treatments, wound care and care plan preparedness. Pennsylvania LPN license and one year of experience with the geriatric population required. This position offers a $5,000 sign-on bonus, ask HR representative for detail.
Responsible for the profession’s unique psycho social perspective to the interdisciplinary evaluation, assessment, plan of care, and ongoing services that occur once LIFE Pittsburgh Participants begin the enrollment process. The Social Worker interventions could include individual Participant contacts; appropriate collateral contact; Participant and family education, assessment and counseling; mobilizing resources, addressing mental health needs as they arise; ongoing care management; advocacy to ensure patient needs are addressed; and in the event, dis-enrollment education and procedures. The Social Worker, in partnership with the Interdisciplinary Team, ensures effective, efficient, and appropriate care in order to optimize the health status and quality of life for our Participants. The Social Worker is knowledgeable regarding social systems and institutions, individual behaviors and can skillfully apply appropriate interventions to meet the needs of LIFE Pittsburgh Participants and family members. Must possess a valid Driver’s license and automobile insurance, Master’s Degree in Social Work, minimum of (1) year geriatric experience in a hospital, long-term care facility or community setting. Must also have experience working with and relating to the geriatric population and their family members.
Registered Nurse $10,000 Sign On Bonus
Under the direction of the Nursing Manager, the RN is responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will promote and maintain the health of LIFE Pittsburgh participants in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Must have minimum of two years’ experience providing direct nursing care and oversight of Geriatric patients, valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting. Personal Care Assistant $1,000 Sign On Bonus
Under the direction of the Community Care Supervisor and Community Care Manager, PCA I and PCA II (CNA) will be responsible for providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL’s. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. This position must have a Valid PA driver’s license, with access to a vehicle and one year of experience as a Nursing Assistant, Home Health Aide is required (CNA’s preferred).

The Quality Analyst is a dynamic position to support program-wide quality program and compliance with Federal, State and PACE regulations. Reporting to the Manger of Quality, this position supports the quality functions such as the data collection, entry, compilation and initial data analysis. This position works closely with the Manager of Quality, management and clinical/operational staff towards to ensure data integrity and reliability. This role will be responsible for compiling internal reports in collaboration with the Manager of Quality and assist on projects focused on quality improvement. Must possess a valid driver’s license and access to a vehicle. Bachelor Degree required (BSN or Master’s Degree strongly preferred), high proficiency in Microsoft Excel including data organization, aggregation, inserting formulas and functions, creating and using pivot tables and creating data visualization etc. as well as proficiency in overall Microsoft Office Suite are all required.
The Van Driver at LIFE Pittsburgh is responsible for providing safe transportation for LIFE Pittsburgh Participants in a timely, competent manner per established protocols and procedures. Ensures participant safety on the vehicle including the use of safety belt restraints, wheelchair and object securement, and monitoring of safe transport behaviors. Responsible for vehicle inspections for safe operations and records. Ensures adherence to scheduled transports for safe and timely movement of participants to and from the center, to external appointments and other locations as indicated. Responsible for strong communication with interdisciplinary team and transportation department to ensure optimal safety, care and service. Valid High School Diploma Required. Must have possession of valid PA driver’s license and a clean driving record from prior two years. One year of experience transporting geriatric, frail or disabled passengers is required.
