LIFE Pittsburgh is designed and developed specifically to sustain independence among frail elderly by offering a coordinated alternative kind of service through a single organization. Our unique health team is able to provide such services as medical care, diagnostic tests, medical equipment, drugs, prosthetic and orthotic devices, nutritional counseling, nursing, social services, physical, occupational, speech and recreational therapists, dietitians, chaplain and transportation staff based on each individual’s needs.
LIFE Pittsburgh may be an appropriate alternative for those patients who may have intensive and complicated medical and social issues, are resource intensive and who may have a low or poor reimbursement rate. Our health team assesses our Participants’ needs and desires, plans and approves services with them, monitors for changes, and provides timely interventions. Primary care and community services are provided through the Day Health Center and through our in-home program according to our Participants’ needs. We arrange a full array of health and social services 24 hours a day, seven days a week, 365 days a year.
Our Medical Director and Primary Care Physicians will work closely with you to ensure that the transition of your patient is done in a caring, smooth and efficient manner. They will spend as much time as is needed with your patient to ensure that they are comfortable with their decision.
