Our mission is to provide a supportive and caring environment
where seniors can flourish, experiencing happiness and a sense of purpose.
How LIFE works
At LIFE Pittsburgh, we are a single source for personalized health care and social services, helping older adults manage daily needs and live safely and meaningfully in their communities.
Our integrated approach helps each Participant receive at-home care and support and, at our Health & Wellness Center, necessary medical services and much-needed social interaction and engagement.
At the heart of LIFE Pittsburgh is a dedicated Interdisciplinary Team (IDT) who takes the time to know you – your story, your routines, and what matters most in your daily life. They listen, share insight, and work together to make sure your care reflects who you are, not just what you need.
Your LIFE Team
Each Participant has a dedicated Interdisciplinary Team (IDT), comprised of multiple clinical and service professionals, including physicians, nurses, personal care assistants, activities staff, therapists, social workers, dieticians, transportation workers.

Your Interdisciplinary Team (IDT) gets to know your health needs, goals, preferences, and daily life. Together with you, they create a care plan shaped around what you need.
The IDT meets regularly to review your care and adjust the plan as your needs change. They coordinate services whether you are at home, at a LIFE Pittsburgh Health and Wellness Center, in the hospital, or in rehab. They also help coordinate appointments, transportation, and follow-up care.
