Step 1

Start with a Conversation

Once eligibility is confirmed, you’ll meet the members of your dedicated IDT – the primary care provider, nurses, social worker, and therapists who will walk alongside you throughout your LIFE Pittsburgh journey. You’ll meet either at home or at one of our Health and Wellness Centers. Together, we’ll build a personalized care plan that supports your health, independence, and daily life.

Your IDT will:

  • Become your ongoing primary care and support team

  • Partner with you to understand your needs, goals, and preferences

  • Create and coordinate a care plan tailored to your daily life

  • Support you throughout your entire journey with LIFE Pittsburgh

Step 2

Understanding Eligibility

If you'd like to move forward, we'll help you determine eligibility for LIFE Pittsburgh. Our team will:

  • We handle the medicaid application process

  • Coordinate the county's functional eligibility assessment

  • Work with your healthcare providers to collect information

  • Keep you updated every step of the way

Step 3

Meet Your Interdisciplinary Care Team

After everything is approved, you'll meet to review your care plan and if you believe LIFE is right for your journey, sign your Enrollment Agreement. Your benefits begin the first of the month following your signature date, and your LIFE Pittsburgh journey officially begins!

Your LIFE Pittsburgh journey begins with a simple, friendly conversation. One of our Program Specialists will take time to understand your needs, goals, and daily challenges, explain how we may be able to support you, and help you explore what the next steps could look like — at your pace.

During this conversation, we will:

  • Learn about your needs, goals, and daily challenges

  • Explain how we may be able to support you

  • Help you understand your options and what next steps could look like.

Disenrollment Information

Participation in LIFE Pittsburgh is voluntary. A Participant may choose to voluntarily disenroll from LIFE Pittsburgh at any time and without cause by notifying LIFE Pittsburgh staff, either verbally or in writing, with as much advance notice as possible. A Participant's voluntary disenrollment becomes effective on the first day of the month following the date LIFE Pittsburgh receives the notice of disenrollment

In accordance with federal PACE regulations (42 CFR §460.164), LIFE Pittsburgh may also initiate involuntary disenrollment under specific circumstances, such as when a Participant no longer meets eligibility requirements, engages in disruptive or threatening behavior, or fails to meet financial obligations, as outlined in applicable regulations.

Until the disenrollment effective date, LIFE Pittsburgh remains responsible for the Participant's care and will continue to provide and authorize all the services needed. LIFE Pittsburgh staff will work with the Participant and their representatives to support continuity of care, including care coordination, referrals, and transition planning to other Medicare, Medicaid, or community-based services, as appropriate.

For complete details regarding both voluntary and involuntary disenrollment processes, please contact LIFE Pittsburgh at 412-388-8040 to speak with a member of the Quality and Compliance Department
