LIFE Pittsburgh and the PACE (Program of All-inclusive Care for the Elderly) model have been described as a nursing home without walls. We arrange a full array of health and social services 24 hours a day, seven days a week, 365 days a year. A health team of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals assesses our Participants’ needs and desires, plans and approves services with our Participants, monitors for changes and provides timely interventions. Primary care and community services are provided through the Day Health Center and through our in-home program according to our Participants’ needs.
If a resident is receiving short term rehabilitation at your facility and you are anticipating a discharge plan, or has been there long term but is requesting that they be given program options so that they may return to the community, you may call to discuss the individual or make a referral to our enrollment department at 412-388-8050. We will visit the individual at your facility, discuss LIFE Pittsburgh with them, and begin our assessment process. We will complete all required referrals and subsequent paperwork, including a referral to the Allegheny County Area Agency on Aging’s Options Assessment Program, as well as a Medical Assistance application which we will submit to the Allegheny County Assistance Office. Our team will work closely with yours to make certain that we gather as much information as possible regarding the individual to ensure a smooth transition back into the community whenever feasible. We are committed to making your responsibility in the process as straightforward and simple as possible.
Our staff members are dedicated to assisting older adults who wish to maintain their independence and age in place in their own homes, in their own community. One significant success story was a female Participant who had resided in a nursing facility for nine years before her family contacted us to evaluate her for our program. We successfully transitioned her back into the community several years ago and she continues to live in the community with her husband, who is also a LIFE Pittsburgh Participant.
