LIFE Pittsburgh provides services to individuals regardless of race, color, creed, religion, sex, nationality, age, sexual preference, handicap or disability.
Once you have enrolled in LIFE Pittsburgh, you agree to receive services through LIFE Pittsburgh. We seek to provide efficient and effective delivery of services for you, and in order to do so, we will provide and/or arrange for your health care needs. Therefore, you will no longer be able to seek services from other providers under the Medicare or Medical Assistance Program. You must receive all needed health care, including primary care and specialty physician services (other than emergency services) from LIFE Pittsburgh. You may be fully and personally liable for the cost of unauthorized services. The health team will coordinate all of your care.
