DIETARY AIDE
Will assist with the coordination & serving of the nutritious & therapeutic meals. Knowledge of therapeutic diets, including portion size, infection control standards & practices. Current Allegheny Co. One year relevant exp. in a healthcare setting. PA Drivers license & access to a vehicle required.
PERSONAL CARE ASSISTANT
Full Time/Part Time - Assist in providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL's. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. Must have a Valid PA driver's license, with access to a vehicle. Experience as a Nursing Assistant, Home Health Aide is required (CNA's preferred).
REGISTERED NURSE
Assessing health needs & promoting/maintaining health through teaching ,counseling & appropriate preventative services. Responsible for the initial implementation of nursing care plans as well as the periodontic re-evaluations. PA RN license, 2 yrs direct geriatric nursing care, valid drivers license/access to vehicle required.
QUALITY ASSURANCE SPECIALIST
Will support the Quality Assurance manager in the implementation of the Quality Management Program. The program will provide retrospective, concurrent and ongoing monitoring of indicators as defined by the Quality Management Standards set by Centers for Medicaid and Medicare (CMS) under the guidance of the Director of Quality an Compliance. Responsibilities will include bringing forth identifiable, meaningful trends and opportunities for improvement through monitoring and documentation of all Grievances and Unusual Occurrences. Will monitor grievances to ensure that resolutions are met within policy guidelines. Prepare, review and submit electronic date (HPMS) to CMS and other regulatory bodies as required. Bachelors Degree, three years in community based, acute or chronic care social or healthcare program, quality assurance and internal auditing experience preferred.
HEALTHCARE DATA ANALYST
This position will be responsible for identifying and manipulating data to meet the needs of our LIFE Pittsburgh's Health Plan Administration functions. This goal will be met by identifying business information needs, extracting data, developing reports and interpreting analytical results. The position will be responsible for data warehousing of large files received in various formats and will also be responsible for file creation, submission and retrieval, as well as reconciliation and error resolution. This individual will manage this process with various third party vendors.
Qualifications include:
Strong database skills with ability to retrieve data from databases on MS SQL Server and to create reports in Crystal Reports, or similar software.
Skilled in compiling data into innovative presentation format.
Ability to examine documents, reports and presentations for correctness and interpreting accuracy.
Advanced Microsoft Office Computer Skills and Strong Reporting Skills. Bachelors Degree required and 1-3 years experience in healthcare analysis strongly preferred.
COMMUNITY CARE COORDINATOR
Responsible for supporting the Community Care Supervisor with the management of the community services provided by Personal Care staff in the center, the participant’s residence or in what ever environment the Participant might be at the moment of needing care ( i.e. temporary living in another residence, SNF, hospital etc.) Along with Supporting management, will also assist in providing personal care needs. Must have valid CNA license, drivers license and access to vehicle. Minimum of 3 -5 years experience as a Nursing Assistant or other comparable/demonstrated experience.
CENTER AIDE
Responsible for assisting Participants with personal/social needs while attending our Adult Day Health Center. Assist with personal care, ambulation, escorting for appointments dinning room service, & activity. HS Diploma & 1 yr experience in Personal care required. Daylight Position, Monday through Friday.
RECREATION SERVICE SUPERVISOR
Plans, organizes, directs and evaluates the overall recreational program to include management of the activities staff and the provision of an individualized program of recreation for participants in collaboration with the overall multidisciplinary team. Must posess a BS in Recreation Therapy, CTRS, 2 years experience in healthcare recreation, valid drivers license and access to a vehicle.
NURSE PRACTITIONER
The Nurse Practitioner at LIFE Pittsburgh is a dynamic position that is responsible for providing community primary health care and developing treatment guidelines for a geriatric population within a multi-disciplinary team environment. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy an active life in the community. We are currently looking for a Nurse Practitioner to work with a Primary Care Physician within one of our three adult day health centers. This candidate must be comfortable providing medical care to a medically complex frail patient with the goal of maintaining the Participant in the least restrictive environment.
PHYSICAL THERAPIST
Will be responsible for the assessment of participants and the provision of physical therapy services. Current PA License and experience with the geriatric population required.
PRIMARY CARE PHYSICIAN
Full time - Through this dynamic care approach, is impacting the lives of frail seniors who's wishes are to remain independent in the communities they live. If your looking for the daily rewards of medical practice and the long term rewards of personal relationships, no other clinical setting will permit such a regular interaction with Participants outside the provider-patient encounter, thus a chance of developing a closer relationship and better understanding of the unique person.
The PACE program creates a comprehensive care environment (social and medical) for those who would typically be homebound, that permit the frail Participant to interface regularly with all of the traditional geriatric service disciplines under one roof (The Interdisciplinary Team). This permits interactive, immediate decision making and rapidly developed and modified Care Plans. The intimate co-mingling of the disciplines often seeing the Participant jointly in the same day, facilitates a merging of perspectives that quite often leads to better, patient centered decisions that might be made in a more traditional, physician driven setting.
The ideal candidate will have excellent clinical skills that cross all aspects of medicine, both in an outpatient and inpatient care settings and must be adapt at communicating and partnering with colleagues and team members as peers. Must demonstrate excellent clinical assessment skills that will lead to determining appropriate diagnostic strategies to best serve the individual needs of the Participant and their individual health care decisions.
Candidate must be a BC/BE Geriatrician or internist with at least 3-7 years experience working with the geriatric population with a family practice or preferably within an Internal medicine environment due to the complexity of the geriatric "at risk" population.
NURSE MANAGER
Full Time, responsible for the operations both in the home & in the Day Health Services of the Clinical Nursing staff of the LIFE Pittsburgh program. Three years exp working in a health facility or community based setting required and 1 to 3 yrs in a progressive management position also required. BS in Nursing or health related profession preferred.
SCHEDULING ASSOCIATE
Will work in collaboration with the LIFE Pittsburgh Community Care Supervisor to ensure that clinical staffing needs are met such as scheduling, coordinating and maintaining in order to meet the needs of our Participants. The Scheduling Associate will coordinate the use of LIFE Pittsburgh Staff as well as external PRN staff to ensure consistent quality of services for both internal and external staffing needs. Will be responsible for the collection, management and reporting of Participant scheduling information as directed by the Community Care Manager and Community Care Manager. Will also assist in collection and tracking of information related to the Personal Care Assistants' job description. Must be willing to take rotating weekend on-call responsibilities, be well organized, show great attention to detail and have Microsoft Excel experience. 1-2 yrs Patient Scheduling and HS Diploma required.
