CHAPLAIN:
Full Time - responsible for providing spiritual support to LIFE Pittsburgh Participants and/or families and assisting individuals to integrate life's events through emotional support and a focus on spirituality. Facilitates Participant attendance of religious services and/or the receiving of sacraments as appropriate. BA required (MA preferred) with appropriate pastoral certification and at least 2 years ministry experience (geriatric exp preferred).
CLINICAL SUPERVISOR:
Full Time - Responsible for the operations both in the home and in the Day Health Services of the Clinical Nursing and Personal Care Services of the LIFE Pittsburgh program and serves as the Home Care Coordinator for LIFE Pittsburgh. Registered Nurse License required (Bachelors Degree preferred) Three (3) years experience working in a health facility or community based setting required. One (1 to 3) years in a progressive management position required.
DIETARY AIDE:
Full Time - Assists the Dietitian with the coordination and serving of nutritious and therapeutic meals and snacks for the participants of LIFE Pittsburgh, using an individualized care plan and in collaboration with the Multidisciplinary Team Care Plan.
FACILITIES MANAGER:
Full Time - Responsible for all facility management including general facility repair and maintenance. Will also coordinate with applicable landlords to resolve tenant/building facility issues. 3-5 years experience with general facility maintenance including minor electrical, plumbing and drywall repairs. LEAD PERSONAL CARE ASSISTANT:
Full Time – Responsible for supporting the Day Center Manager with the management of the community service provided by Personal Care staff in the center and at the residence of the Participants of LIFE Pittsburgh. Along with supporting management, will also assist in providing personal care needs. Must be CNA certified.
PERSONAL CARE ASSISTANT/CNA:
Full Time/Part Time - Personal Care Assistants assists in providing care to meet the needs of the frail elderly to promote optimal health, and maximize independence and self-care. Will be responsible for providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL's. Performs personal care services for Participants at their place of residence, and/or the Day Health Center. Must have a Valid PA driver's license, with access to a vehicle. Experience as a Nursing Assistant, Home Health Aide is required (CNA's preferred).
PURCHASING COORDINATOR:
Full Time – Responsible for directing and administering purchasing function at LIFE Pittsburgh. Process includes identifying and negotiating with vendors to ensure best price contract terms, timely delivery and a high level of quality for all supplies. Coordinates issuance of purchase orders, receiving, distribution and storing of items. Works with clinical and rehab staff to determine specific patient needs. Experience - 3-5 years purchasing or materials management experience in the medical industry experience required. Knowledge of durable medical equipment purchasing and inventory especially valuable.
REGISTERED NURSE:
Full Time - Responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will promote and maintain the health of LIFE Pittsburgh participants in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting.
TRAINING SUPERVISOR:
Full Time - Responsible for the assessment, development, implementation & evaluation of formal and informal education needs in collaboration with the management team as defined by CMS and Department of Public Welfare regulations. BA required & 3-5 years education/training exp. (healthcare pref).
RECREATION ASSISTANT:
Full Time - Assist in activities for our center based geriatric program. Healthcare experience preferred. Generally Mon - Fri.
CERTIFIED OCCUPATIONAL THERAPY ASSISTANT (COTA):
Full Time – Responsible for promotion and maintenance of the functional independence and wellbeing of participants in the Adult Health Center and at home. Under the supervision of the OTR, will assist with assessment, treatment and therapeutic interventions designed to keep participants in the community. Geriatric experience required and a PA license as an Occupational Therapy Assistant.
